Can I register my family members on the portal?

Only BayCare patients who have provided an email address will receive an email invitation to register with the BayCare Patient Portal. If you have a power of attorney or are the patient’s representative, inform the clerk at the time of admission. Please call (888) 499-9003 if you have any questions or need assistance registering with the BayCare Patient Portal.

I am a new patient at BayCare Medical Group. Can I access my new patient forms on the portal?

Pre-visit forms can be found on the portal. Questions can be directed to your supplier. Fill out the forms completely as soon as possible after accessing them. You cannot save and finish them at a later time. You should complete the intake forms seven business days prior to your appointment.

Can I submit emergency medical requests through the patient portal?

No. If you have a medical emergency, call 911 right away. Do not send urgent or emergency messages through the Patient Portal.
